Lakeland Skilled Nursing and Rehabilitation Center has had the same name, serving North East Indiana residents, for over 40 years. When we first opened we were admitting 6 people per year. As with any successful and thriving enterprise we have evolved to meet the needs of 'today's generation'.

We have become a rehabilitation station, with a focus on getting people recuperated and back 'out there' enjoying their lives as independently as possible. We continue to provide skilled nursing services for the long-term care resident as well. Our programs include stroke rehabilitation, Alzheimer’s care and out-patient care.

Our goal is to help individuals achieve their highest possible functioning level and to assist them in returning home. Our short term rehab unit (opened in 2012) is separate from the nursing home and includes a new, larger therapy gym, separate rehab dining, separate entrance and new “virtually private” rooms.

Our nursing home is fortunate to have a Medical Director, Dr. Vijay Kamineni and Nurse Practitioner, Stefanie Shire, exclusively devoted to Lakeland. They are in personal contact with us on a daily basis, providing quality care for our residents. Our staff and residents are supportive of each other as reflected in the Covenant Care philosophy, "We Are Family, Serving Families".
