Gilroy Healthcare & Rehabilitation Center is looking to find a full-time Admissions Coordinator to be part of admissions team.  This is a full-time 32 hours per week position, including Saturdays and Sundays. 

 

 We are a 132 bed facility with a diverse mix of patients located in Gilroy, CA. (about 20 minutes from San Jose!) If you answered “yes” to the questions above we want to meet with today!  We are Families serving Families and all our staff are a valued piece of that vision.

POSITION SUMMARY: The primary purpose of this position is to assist and lead the facility's admission process.

 

KEY JOB FUNCTION:  

  • Adequately monitors (i.e. Audits) Admissions for compliance and opportunities for improvement.
  • Maintains timely, effective communication with facility and acute hospitals regarding bed availability; pending admits & discharges.
  • Collaborates timely and accurate admissions processing.
  • Facilitates and maintains real-time data on referrals, admissions and discharges in Salesforce as required.
  • Facilitates accurate identification and data entry of Responsible Party, Physician, Pharmacy, Mortuary, etc.
  • Ensures tours are managed timely and in accordance with established CC standards and facility plan.

 

 

 

SCHEDULE DETAILS:

  • Full Time - 4 days per week
  • Saturdays and Sundays 10 AM - 6 PM
  • Weekday days and times are flexible 

 

 

 

 

Minimum Requirements

  • Must have, as a minimum 2-3 years medical experience in long-term care.
  • Must possess a high school diploma or GED.
