POSITION SUMMARY: The purpose of this position is to provide assessment and psychological interventions to residents.

 


 

 KEY JOB FUNCTION:

  • Understands the MDS Process and accurately assesses, documents and provides interventions for assigned case load.
  • Ensures accurate, on-going assessment and care planning for identified psychological needs for assigned case load.
  • Facilitates discharges that accurately meet the needs wishes and goals of the resident and family.
  • Attends Walking Rounds, PPS meetings, family and resident conferences as assigned.
  • Effectively communicates with clinicians and caregivers to identify clinical or service problems and create solutions.

Minimum Requirements

  • At least 1 year of experience in Social Work.
  • Must possess an Associate's Degree in Social Work or other related field.
  • Must meet all license, certification, training or credentials as required by the State.
