    Covenant Care is seeking a Director of Nursing to lead and manage the Nursing Department and utilize available resources to provide quality, caring services to all residents and patients, and achieve desired clinical and financial results.  Los Altos is a beautiful 152 bed facility, strong short term rehab, cardio/pulmonary specialty, wound care, great nursing support - fantastic team!  The Director of Nursing will collaborate with physicians, staff, and partnering hospitals to care for high acuity patients.



    In 2018, Covenant Care admitted 16,100 patients to our skilled nursing facilities in California, Nevada and Illinois. We sent home a higher percentage of patients than our peers and we outperformed them in all 5-star ratings. 100% of our eligible facilities achieved either a Gold, Silver or Bronze National Quality Award.     Minimum Requirements

    The ideal candidate has a current California RN license and preferably a Bachelor’s in Nursing.  Experience as a supervisor in a hospital or long-term care facility, with working experience in rehabilitative and restorative nursing practices.  Working knowledge of state and federal laws to guide the practices and processes of the facility and team; as well as presentation of data to partnering hospital systems.  Knowledge and ability to promote clinical revenue improvements through effective participation in the PPS care management process, and through Part B and other services (therapy).  Knowledge of best practices and standards to achieve and maintain the 5-star facility rating is a plus.
