    POSITION SUMMARY: Responsible for generating, managing leads and converting business in primary referral hospitals. Responsible for facilitating a smooth discharge process for patients transitioning from a hospital setting to our centers.  The Clinical Liaison will develop strong relationships with key referral hospital personnel, educate on Covenant Care services, and identify new opportunities for partnering and growth in order to increase referrals.

     


     

     KEY JOB FUNCTION:

    • Manages a personal set of accounts and contacts at key referral sources.
    • Documents all sales call and patient assessments in Covenant Care CRM system in accordance to the policy and procedures.
    • Develops referrals from hospitals' social services and discharge planning departments, physicians, case managers, insurance companies and other referral sources.
    • Makes outside sales calls to hospital social workers, physicians, hospital discharge planners and administrators, to understand the need for program services and to communicate services offered which meet these needs to obtain referrals of patients with the goal of developing new business.
    • Complete daily rounds in designated hospitals, obtaining new inquiries and evaluating patient's individual care needs prior to admission, identify new patients and direct to appropriate center.
    • Troubleshoots escalation issues with the DSM to determine ways to meet the needs of patients.

    Minimum Requirements

    • Must be a Registered Nurse, LPN or LVN.
    • Must have 2+ years experience interacting and working with hospital based clinicians (physicians, nurses, discharge planners, and social workers).
    • Must have 2+ years experience in an outside sales and marketing role, with demonstrated experience in making sales calls to healthcare professionals.
    • Must meet all license, certification, training or credentials as required by the State.
