    POSITION SUMMARY: The primary purpose of this position is to assist the Social Services Director in identifying and providing for each residents social, emotional and psychological needs.




    • Coordinates and sets up appointments including Dental, Podiatrist, Ophthalmologist, Psychiatrist.
    • Coordinates and arranges transportation, where applicable.
    • Arranges room changes, assist resident/family with adjustment to change.
    • Keeps calendar for assessments, meetings and other clerical duties as needed, and schedules meetings for residents, staff, families.
    • Assist in development of a volunteer program and assist in facilitating family group meetings.
    • Arranges for DME and home services at time of discharge, and follows up on residents possessions after discharge.
    • Provides timely support for reduction of psychotropic’s and restraints as assigned.

    Minimum Requirements

    • Must possess a high school diploma or GED.
    • Must meet all license, certification, training or credentials as required by the State.

    Preferred Requirements

    Must have a social services degree.
