    POSITION SUMMARY: The primary purpose of this position is to lead and manage the facility's admission process.




    • Adequately monitors (i.e. Audits) Admissions for compliance and opportunities for improvement.
    • Maintains timely, effective communication with facility and CP regarding bed availability; pending admits & discharges.
    • Ensures bed availability is communicated to Centralized Placement daily, and responds to Centralized Placement alerts timely to facilitate prompt conv opportunities.
    • Collaborates with ASM/CP to facilitate timely and accurate admissions processing.
    • Facilitates and maintains real-time data on referrals, admissions and discharges in Salesforce as required.
    • Facilitates accurate identification of Responsible Party, Physician, Pharmacy, Mortuary, etc.
    • Ensures tours are managed timely and in accordance with established CC standards and facility plan.

    Minimum Requirements

    • Must have, as a minimum 2-3 years medical experience in long-term care.
    • Must possess a high school diploma or GED.
