    • Person must have access to placement services local and throughout California.
    • Ability to complete discharges from start to finish with minimal assistance from facility.
    • Assist in identifying residents who would benefit from a discharge to an appropriate lower level of care.
    • Provide a weekly report of activities via email that have been conducted to facilitate discharge for each resident.
    • Meet with each facility IDT/BLT monthly or more frequently at Executive Directors request to receive new information and provide updates or requests for current discharges in process.

    Preferred Requirements

    • Bachelor degree in Social Services or licensed nurse required.
