    Courtyard Healthcare Center  is one of Covenant Care’s 55 locations.  We are located in Davis, CA.  Courtyard has 112 beds and a blend of both Short Term care and Long Term care patients. Our facility also offers the Rehabilitation services.  We are looking to hire an experienced Minimum Data Set (MDS) Manager to join our team!  


    This is a great growth opportunity.  The goal is to train with our internal team and advance to the RAID (Resident Assessment Instrument Director) within 3-6 months of hire.  The RAID is part of our Business Leadership Team (BLT) which is unique to Covenant Care and allows you to participate with all the leaders in developing the strategy and vision for your building.  



    The primary purpose of the MDS Manager is to manage the MDS function and processes to:

    (1) maximize clinical outcomes for patients and residents, and

    (2) ensure timely, accurate and complete clinical and support documentation to enable the facility to be fully reimbursed by government payer sources for all services provided, by accomplishing the specific business goals.

    • Coordinates the completion of assessments per the OBRA and PPS schedule as appropriate.
    • Timely communicates findings and recommendations to Director of Nursing and Executive Director.
    • Assist in developing a timely, accurate, and comprehensive care plan for each resident that is individualized with measurable objectives.
    • Completes timely transmission of MDS assessments per Company standards.
    • Timely and accurately corrects rejected MDS transmissions.
    • Analysis and reporting of trends to the QA Committee monthly with suggestions for quality improvement.
    • Supervises additional department staff to ensure completion and accuracy of MDS completion and assigned tasks.

    Minimum Requirements

    • Must have current Registered Nursing (RN) license.
    • Preferred two years experience in Long Term Care.
    • Preferred at least two years experience in MDS.
    • Must meet the general health requirements set forth by the policies of the facility or department which may include a medical and physical examination.
